Office Closed June 20-24, 2022
ANNOUNCEMENT
Our office will be CLOSED on Monday, June 20 in recognition of Juneteenth. Our staff will not be available on this day.
Our office will be CLOSED from Tuesday, June 21 through Friday, June 24 for renovations. Our staff will still be available during this time via email and will be checking the office voicemail periodically.

You may also utilize the blue tote on the ground outside our office to pick up forms or test kits.
