York County 4-H Field Trip Form

Please complete the Field Trip Form at least 2 weeks prior to the field trip date. If the extension office is not aware of the field trip, then volunteers are NOT covered by University of Maine liability insurance. A field trip is any activity located anywhere outside the regular club meeting space. Examples include tours, hikes, sledding, or similar.
