Adult Chaperone Confirmation

Thank you for registering for the 2019 4-H@UMaine: Connecting Kids to Campus! Check your email for a copy of your confirmation.

Chaperones must fill out and send the following completed forms in addition to your registration:

Maine 4-H Health Form (PDF)
Release and Assumption of Risk Form (PDF)

If you registered a youth or youths in addition to yourself and were not directed to the payment page, you can pay now by going to our secure payment portal.

All forms must be received by Greg Kranich by May 3, 2019 before registration is considered complete.
