There are several forms that are due annually for all 4-H clubs. They are available here as Word documents, PDFs, or as online forms. Please contact the Extension office if you would like a hard copy of the form.

York County 4-H Club Grant Application (Word | PDF)
York County 4-H Club Grant application ideas should tie into the 4-H program priorities including community learning projects, philanthropic projects that engage members in the community, and/or new creative educational ideas. Youth are expected to fundraise or seek donations to cover expenses, with the Club Grant making up the difference, up to $100. The event/activity should be available to all enrolled 4-H members to participate. Age restrictions for participants may be applied when appropriate. Individual clubs/groups may not apply for more than one grant in a 12 month period. Decisions may take up to four weeks from date of submission.
Eligibility:
- Club is based in York County
- Club has existed for at least one 4-H year (Oct 1 – Sept 30)
- Club has a minimum of four enrolled 4-H members from at least three different families
- Must have an EIN provided by the Extension Office and a bank account maintained by the club leader
