Some of the links below point to PDF files on the MyCampus Portal. When accessing Portal documents, you need to be logged into the MyCampus Portal using your Gmail login info. Another way to get to the files is to go to Extension’s Community on the UMS myCampus Portal, then navigate to Eat Well, then Forms, then the form you want.

Color graduate certificates with blue or red text will change color when you type in the participant’s name and then tab off the name field.
